This information request will help facilitate your upcoming trust examination. Please complete each item by providing thorough, narrative responses in the space at the end of each section or by providing documents attached and labeled. An officer of the institution should review and confirm that all responses are complete and accurate.
Management
1. Trust Officer’s Questionnaire (form provided), certified by the senior trust officer or another senior bank officer.

2. Trust Department Statement of Assets and Liabilities (form provided) certified by the senior trust officer or another senior bank officer.

3. Provide an organizational chart of the department. Provide a list of offices in which trust or wealth management activities are conducted. Describe the involvement of the Board of Directors in establishing business goals, reviewing actions and decisions of officers and committees, and ensuring compliance with applicable law.

4. A list of all your senior officers showing title, salary, bonus, telephone number, and office location. Include a resume and job description for each officer that lists principal responsibilities and the amount of time devoted to each. For all other personnel provide the total number of employees and their combines salaries. Discuss whether staffing is appropriate or whether there will be hiring or downsizing of staff.
