10 MANAGEMENT A. Completed Trust Officer’s Questionnaire, General Information Questionnaire, and Trust Company Statement of Assets and Liabilities. B. Access to the meeting minutes of the Board of Directors (“Board”) and Trust Company (“Company”) related supervisory committee(s) pertaining to all trust activities since the previous examination. C. Access to all of the Board meeting “Packages” and/or support materials since the previous examination. D. Organization chart of the Company. E. Current strategic plan for the Company, along with the current “Resolution Plan” (also known as the “Living Will”). F. Formal management succession plans. G. Chronological resume of all Company officer if they were hired since the last examination listing education and degrees awarded, and work history, including names of institutions where employed, title, principal duties, and dates promoted. H. Access to written policies and procedures for corporate and fiduciary activities. Please indicate the date on which the Board approved the policies and procedures. I. Most recent Regulation R calculation and any accompanying policy or other documentation evidencing compliance with applicable exemptions/exceptions. J. Provide the following audit program information: 1. Copies of the Company’s audit policy, risk assessment and audit schedule and programs for all trust accounts (such as personal, employee benefit, and corporate) and any other Company activities. 2. Copies of all Company internal and external audits performed during the last 24 months, as well as the related issues tracking report. 3. Copy of the most recent engagement letter. K. Provide the following compliance program information: 1. Copy of the Company’s compliance program policy and procedures. 2. Copy of the most recent compliance report to the Board. L. Provide the following risk management program information: 1. Copy of the current risk management program policy and procedures. 2. Copy of the most recent risk assessment “Dashboard” or similar risk summary report to the Board. 3. Copy of the supporting documentation that formulated the most recent Dashboard.
