Trust Officer’s Questionnaire (Continued)

13. During the last calendar year, did all trust personnel take annual vacations, which required them to be continuously absent from their duties for a minimum of five consecutive business days? If not, give the name of the person and indicate principal duties performed.

14. List all extensions of credit that have been made from corporate assets of the department since the previous examination.

15. List any extensions of credit and corresponding balances which since the previous examination have been renewed or have had their due dates extended (a) without full collection of interest thereon; (b) by acceptance of separate notes in payment of interest; (c) by capitalization of interest to the balance of the note. For all listed loans, state whether (a), (b), or (c). In none, so state.
