State Agency completes self-evaluation questionnaire (SEQ)

SEQ Completion

Review Team of subject matter experts visit Agency to evaluate the agency

Onsite/Off-site Review

In between five-year reviews, the agency completes annual review, and provide updates on how they are addressing the recommendations The final report is provided to the agency with recommendations and suggestions to meet accreditation standards A report goes to the Performance Standards Committee for accreditation decision
